Accessing a placement at the Seeds of Change can be done in various ways and we are always happy to offer help and advice, either call or email us.
Accessing a placement at the Seeds of Change can be done in various ways and we are always happy to offer help and advice, either call or email us.
We will need the following information:
A referral form completed by the funding body, this includes information about the attendee, their current situation and what the referrers hopes to achieve by the young person attending. We use this information to agree impact measures, then from this the coach identifies the learning objectives, these are the specific stepping stones for that young person on their journey.
We use the SDQs questionnaire to assess and track the progress of the young people who attend. These need to be completed by an adult in the young person’s life, and the young person (we can do this when the young person attends)
We require consent from the parent or carers of the young person.
We also request any other relevant document such as risk assessments or EHCP. We are happy to attend review meetings and any other professionals meeting that might be relevant to supporting the young person.
At The Seeds of Change we take our safeguarding responsibility very seriously and all concerns will be brought to the attention of the referrer. We do ask that professionals share relevant concerns ahead of time with us allowing us to support the young person.
